How can I receive treatment at Midwest HealthStrategies?

Discuss with your physician, your injury or problem and your desire to receive therapy.

Your physician will need to fill out a script with your name, diagnosis, and requested treatment.

You can bring in the script or call (765) 751-2555 or 1-866-669-0691 to one of our many Midwest HealthStrategies locations and schedule an appointment.

Midwest HealthStrategies will then verify your insurance coverage and them explain your benefits and schedule an appointment for you to be treated by a therapist.

Our organization will remain in communication with your physician during and after treatment regarding your progress.
